Begin by submitting The Bagby Way’s request form or contacting The Bagby Way directly

COMPLETE THE BAGBY WAY INTAKE

A team member will collect the basic information needed to understand the assistance being requested. The Bagby Way maintains its own intake process, records, consent forms and privacy notices.

IDENTIFY SOCIAL NEEDS

A Community Health Worker or other authorized team member may complete a social-needs screening to identify priorities, available resources and possible next steps.

RECEIVE RESOURCES AND REFERRALS

Based on the client’s needs, The Bagby Way may provide information, nonprofit resources or referrals to community organizations and other service providers.

CHOOSE WHETHER TO RECEIVE A CLINICAL REFERRAL

If services outside The Bagby Way’s nonprofit scope are needed, the client may choose whether to receive a referral to a separate, qualified clinical or wellness provider.

CONSENT TO A WARM HANDOFF

A warm handoff will occur only after the client gives consent. The Bagby Way will share only the minimum information necessary to support the referral.

FOLLOW-UP

When authorized by the client, The Bagby Way may follow up to help the client understand next steps or identify additional community resources.
