CASE MANAGEMENT FOR CHILDREN AND PREGNANT WOMEN (CPW)

Helping eligible children and pregnant women connect with health-related services and support.

 The Bagby Way is preparing to provide Case Management for Children and Pregnant Women (CPW), a Texas Medicaid service designed to help eligible children and pregnant women with health-related needs access medical, social, educational and other appropriate services.


CPW is a specific Texas Medicaid case-management benefit. It is separate from The Bagby Way’s general Community Health Worker, food-assistance and community-resource services. Not every service offered or coordinated by The Bagby Way is covered by Medicaid.

WHO MAY QUALIFY?

Case Management for Children and Pregnant Women may be available to:


CHILDREN

Children from birth through age 20 who receive Texas Medicaid and have a health condition or are at high risk for developing a health condition.


PREGNANT WOMEN

Pregnant women of any age who receive Texas Medicaid and have a health condition, health risk or high-risk condition.

Meeting one of these descriptions does not automatically establish eligibility. Medicaid enrollment, health-related need, service eligibility, provider availability and applicable health-plan requirements must be verified through the official intake process.

INITIAL INTAKE

The intake process helps determine whether the child or pregnant woman may qualify for CPW and whether case-management services are needed.

FAMILY NEEDS ASSESSMENT

A case manager works with the client, parent or legal guardian to understand medical, educational, social and other health-related needs affecting the client’s health and well-being.

INDIVIDUAL SERVICE PLAN

When a client qualifies, the case manager develops a service plan identifying needs, goals, referrals and appropriate next steps.

REFERRALS AND COORDINATION

The case manager may help identify and coordinate appropriate medical, dental, behavioral-health, educational, social and community services.

FOLLOW-UP

The case manager follows up as needed to help determine whether the service plan is being carried out and whether the client’s needs or circumstances have changed.

CPW case management does not replace medical care, prenatal care, behavioral-health treatment or emergency services. Each healthcare provider, Medicaid health plan, government agency and community program determines its own eligibility, availability and service requirements.

 SELF-REFERRAL

An adult client, parent or legal guardian may contact The Bagby Way directly to request information or assistance with a CPW referral.


PROFESSIONAL REFERRAL

Healthcare providers, social workers, schools, Medicaid health plans and community organizations may submit a referral with the appropriate permission from the client, parent or legal guardian.

WHAT HAPPENS AFTER A REQUEST?

  1. Submit the CPW Assistance and Referral Request Form.
  2. A team member reviews the request and determines whether additional information is needed.
  3. The Bagby Way contacts the client, parent or legal guardian using the authorized contact information.
  4. Medicaid status, health-plan information, eligibility and applicable authorization requirements are verified.
  5. If the person may qualify, the official CPW intake process is explained and arranged.
  6. If The Bagby Way cannot accept the referral, the person may be directed to Texas Health Steps, their Medicaid health plan or another authorized CPW provider.

Submitting a request is not the same as enrolling in CPW and does not guarantee acceptance or services.

YOUR INFORMATION AND CONSENT

 

The Bagby Way will collect only the information reasonably necessary to review the request, contact the client or authorized representative and complete the approved referral or intake process.

For a child, a parent or legal guardian must be identified. Information will not be shared with outside providers or organizations without the appropriate authorization, except when disclosure is permitted or required by law.

Please do not send Social Security numbers, complete Medicaid identification numbers, insurance-card images, medical records or identification documents through ordinary email or an unsecured website form. Secure instructions will be provided if additional records are required.

COVERAGE AND ELIGIBILITY NOTICE

 Case Management for Children and Pregnant Women is a Texas Medicaid benefit. Medicaid enrollment, CPW eligibility, medical necessity, provider participation, health-plan requirements and any required authorization must be verified before Medicaid-covered services are provided.

Submitting an inquiry or referral does not guarantee:

• Medicaid eligibility
• CPW enrollment
• Acceptance by The Bagby Way
• Health-plan authorization
• Medicaid payment
• A particular referral, benefit or service

The Bagby Way does not make Medicaid eligibility or coverage decisions on behalf of Texas Health and Human Services or a Medicaid health plan.
